Community-Based Outpatient Clinic

If you live close to Jackson and would like to be treated at the Jackson clinic, you must first make application with the Memphis VA Medical Center.

Driving Directions

The Jackson Community Based Outpatient Clinic is located on Old Hickory Blvd in Jackson. 

From the West: Take US 421 (North Parkway) East. Turn left onto Rosenblum Drive.  Turn right onto Old Hickory Blvd.

From the South: Take US 45 North.  Turn left onto Old Hickory Blvd.  The clinic is on your left in approximately 1/2 mile.

From the North: Take US 45 South. Turn right onto Old Hickory Blvd. The clinic is on your left in approximately 1/2 mile.

From the East: Take US 421 (North Parkway) West. Turn right onto US 45 North.  Turn left onto Old Hickory Blvd. The clinic is on your left in approximately 1/2 mile.
